Survey Binder is operated by Audrow Nash LLC ("Survey Binder", "we"). These terms apply when you use surveybinder.com or buy a Survey Binder service. By purchasing, you agree to them.
1. What we provide
Survey Binder provides readiness reviews, document templates, checklists, due-date tracking, and reminders to help home care agencies prepare for state surveys and license renewals.
2. Not legal or regulatory advice
Our services are informational and organizational. They are not legal, regulatory, or clinical advice, and we are not your attorney or consultant of record. You are responsible for your agency's compliance with all applicable laws and rules.
3. No guaranteed outcome
We don't guarantee that any license will be issued or renewed, or that any survey or inspection will be passed without findings.
4. Your review
You must review and adapt every document we provide to your agency's actual operations before adopting it. Rules can change after the date shown on a document.
5. No health information
Don't send us client or patient health information. Our services aren't designed to hold it, and we don't enter into business associate agreements.
6. Payment, renewal, and cancellation
Prices are shown at checkout. The monthly plan renews automatically each month until you cancel. Cancellation and refunds follow our refund & cancellation policy.
7. Limitation of liability
To the fullest extent permitted by law, our total liability for any claim is limited to the fees you paid us in the 12 months before the claim, and we aren't liable for indirect or consequential losses, including fines, lost licenses, or lost revenue.
8. Changes
We may update these terms. We'll post the new version here with a new date, and email active subscribers about material changes.
